Your First Visit

We encourage our patients to arrive 10 minutes early to their first appointment to give them time to fill out our New Patient Form.

NEW PATIENT FORM*

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

Your dentist will begin by reviewing your medical and dental history with you and then will complete a comprehensive dental exam.

This examination will include an assessment of your teeth, gums, jaw and bite and will also involve an oral cancer screening and any required x-rays.

Treatment Plan

Based on your medical history and the examination, your dentist will then develop a long-term dental treatment plan for you.

They will also take the time to answer any questions you might have about your treatment options and dental health.
